Navio Hospital Abaré: Uma novela sem fim

O Projeto Saúde e Alegria (PSA), ONG sediada em Santarém (PA), sempre procurou somar esforços a políticas públicas para assegurar o direito à saúde e reduzir os níveis de exclusão das populações ribeirinhas de áreas remotas da Amazônia.
Na busca pela construção de um modelo de atenção básica resoluto e adaptado, um passo foi dado, em 2006, com a implantação do navio-hospital Abaré, que viabilizou o acesso regular a serviços assistenciais para 15 mil ribeirinhos de mais de 70 comunidades das duas margens do Tapajós, nas zonas rurais dos municípios de Santarém, Belterra e Aveiro.

Capítulo 1: O que pensávamos ser o mais difícil: a construção de uma solução de saúde básica para áreas remotas da Amazônia
Implantado pelo PSA em parceria com as prefeituras e apoio da ONG holandesa Terre Des Hommes (TDH), o Abaré funcionou nos moldes de um PSF (Programa Saúde da Família) itinerante, com serviços de saúde da criança, saúde oral, imunizações, pré-natal, PCCU, planejamento familiar, atendimentos médicos, ambulatoriais, exames de rotina e pequenas cirurgias.
Além de médicos, dentistas e profissionais de enfermagem, uma equipe de arte-educadores acompanhava as rodadas, realizando dinâmicas ludo-pedagógicas de mobilização e prevenção, como ações integradas e complementares às assistenciais.
Ao longo dos anos, os esforços conjuntos começaram a dar resultados, com uma rotina de retornos a cada 40 dias, mais de 20 mil procedimentos de saúde/ano, obtendo significativa melhora dos indicadores e resolutividade de 93%: somente 7 a cada 100 pacientes sendo encaminhados, em média, aos centros hospitalares urbanos.
Essa iniciativa ganhou visibilidade além das fronteiras tapajônicas, com diversos prêmios, publicações, visitantes ilustres, como o Príncipe Charles, ministros, governadores, entre outras autoridades, que reconheceram, no polo santareno, a capacidade de articular os setores público, privado e a sociedade civil na formulação de soluções concretas passíveis também de replicação em outras regiões.
Capítulo 2: A utopia vira realidade: o modelo de saúde construído é transformado em política pública
Em 2010, a experiência foi reconhecida como política pública nacional, tornando o polo Tapajós referência em boas práticas de saúde ribeirinha. Ótima notícia para enfrentar o difícil desafio assistencial nas zonas rurais amazônicas, com municípios do tamanho de países, grandes distâncias, populações dispersas e custos logísticos superiores aos padrões nacionais até então oferecidos pelo SUS.
A política de Saúde da Família Fluvial -lançada por meio da Portaria 2.191 de 3 de agosto de 2010, e aprimorada no ano seguinte através da Portaria 2.488 de 21 de outubro de 2011- instituiu novos arranjos e critérios para implantação e financiamento de barcos de atendimento a populações remotas com vistas a apoiar os municípios de toda Amazônia Legal e Pantanal, sua área de abrangência. Uma grande conquista, pois permitiu disseminar o que se semeou no Tapajós para outras regiões necessitadas.
Atualmente, mais de 50 novas embarcações assistenciais vêm sendo implantadas e custeadas com apoio financeiro federal. O primeiro deles foi o próprio Abaré, que em dezembro de 2010 foi credenciado pelo ministério como a primeira Unidade Básica de Saúde (UBS) fluvial do país, sendo integrado ao SUS. A partir daí, os municípios tiveram melhores condições para assumir a sua gestão, liderada por Santarém, com verbas anuais de R$ 600 mil para o custeio -padrão previsto na portaria por embarcação.
Desde então, há mais de dois anos, apesar de o Projeto Saúde e Alegria ter deixado a linha de frente das operações no Tapajós, continuou apoiando as comunidades no controle social, na educação/prevenção em saúde, assim como na gestão pública, quando solicitado.
E, com base nos acúmulos e lições aprendidas, passou a se dedicar na disseminação do modelo de saúde fluvial para outras regiões, sobretudo nas áreas de abrangência da portaria ministerial. Um exemplo foi a aquisição, pelo PSA, de um segundo barco, o Abaré II, repassado à gestão pública para viabilizar os atendimentos junto a mais de 10 mil ribeirinhos da bacia do rio Arapiuns, também no Baixo-Amazonas.
Capítulo 3: A novela Abaré edição 2012: do Brasil (também colônia holandesa) ao Brasil emergente
Para o PSA, o Abaré simbolizava o sucesso de um modelo de saúde cujos ribeirinhos do Tapajós eram também partícipes de sua construção, tanto que até escolheram e foram responsáveis por seu nome de batismo -Abaré significa amigo cuidador, em Tupi.
Apesar dos avanços, no começo de 2012, a ONG holandesa TDH, proprietária do barco, decidiu, de forma abrupta, retirá-lo do Tapajós e deslocá-lo para outra área, descumprindo assim a expectativa, desde o início, da parceria pela doação da embarcação à região.
Os rumores indicavam que o Abaré passaria a servir uma outra região por meio de uma organização do Estado do Amazonas. Nada contra novos apoios e projetos, desde que somem e não se inviabilizem. O fato é que, sem barco substituto, sua saída inesperada do Tapajós significava a interrupção dos atendimentos aos ribeirinhos, que já vinham funcionando por meio de um arranjo público-privado custeado por verbas federais.
Por mais que se reconheça o apoio determinante da TDH ao longo dos anos, quando a ONG holandesa deu a impressão de entender o Abaré como meio de negócio, e não como meio de saúde, a parceria se desfez.
Iniciou-se então uma grande campanha pela permanência da embarcação – #FicaAbaré-, com manifestações dos ribeirinhos do Tapajós amplamente apoiadas e debatidas nas mídias, blogs e redes sociais, um exemplo de exercício efetivo de cidadania, que gerou comoção em toda região, com a união de comunidades, instituições, municípios e governo federal.
O desdobramento dessa campanha, após intensos embates, inclusive pelas vias judiciais, tomou rumo por uma solução definitiva com tratativas iniciadas em meados de 2012 para a compra do Abaré, envolvendo a Prefeitura de Santarém e os proprietários holandeses, com aval do Ministério da Saúde.
Recursos federais foram assegurados para sua aquisição e repasse à Secretaria Municipal de Saúde (Semsa) de Santarém. Com isso, além da continuidade estável da assistência no Tapajós, abria-se um enorme potencial para diversificação dos serviços, incorporação de novos parceiros e programas, incrementando o papel social da embarcação.
Negociações vinham avançando até o final do ano passado no sentido de transformá-lo também em um barco-escola, com o envolvimento de universidades como a Uepa e USP, um indutor para interiorização da medicina na Amazônia, na forma de um laboratório e polo difusor de boas práticas para replicação nas outras regiões contempladas pela Portaria 2.488/2011, entre outras benesses de interesse estratégico do Ministério da Saúde.
Capítulo 4: A novela Abaré edição 2013: descontinuidades entre PTs e PSDBs põe a iniciativa na UTI
Por ironia do destino, os momentos mais decisivos para o fechamento do negócio calharam de acontecer entre o período das campanhas eleitorais e a virada de ano (e das gestões municipais, como no caso de Santarém, saindo o PT e entrando o PSDB), o que contribuiu para morosidade dos processos.
Em dezembro passado, os instáveis holandeses decidiram não vender mais o barco. A antiga administração de Santarém deliberou então pela desapropriação e obteve sinalização favorável do Ministério da Saúde, embora em comunicado datado de 31/12/12 às 00:05:23, já no marco das festividades de fim de ano.
Com as mudanças dos governos municipais, os processos em curso foram paralisados, pois dependiam de deferimento das novas gestões para seguirem adiante.
No início de 2013, a Prefeitura de Santarém fechou acordo para o aluguel do Abaré com a TDH, passando a pagar à organização holandesa pela disponibilidade da embarcação por apenas dez dias mensais, ou seja, somente para o período de atendimentos na margem esquerda do Tapajós -comunidades do seu município-, excluindo as localidades do outro lado do rio, pertencentes a Belterra e Aveiro.
Diante disso, mais de 5 mil ribeirinhos da margem direita do Tapajós se encontram sem assistência desde o ano passado. Por serem municípios menores, ele não têm condições financeiras para bancar os custos de aluguel, o que levou Belterra a negar a proposta de locação do barco também oferecida a ela pelos holandeses no início de 2013.
O acordo pode agravar ainda mais a situação. Além da suspensão no avanço das tratativas com Brasília para aquisição do barco, Santarém corre sérios riscos de perder as verbas federais que vêm sendo repassadas há mais de dois anos através da Portaria Ministerial do Tapajós, que tem abrangência intermunicipal e exige por lei o mínimo de 24 dias-campo/mês, período superior aos dias do aluguel acertado.
Como previsto, aconteceram novas manifestações dos ribeirinhos, pedindo providências, alinhamento dos municípios e agilidade na retomada do diálogo com Brasília.
Cenas dos próximos e últimos capítulos: Municípios e controle social definem o final da novela
A saúde, por mexer com a vida das pessoas, deveria estar acima de qualquer interesse de grupos ou partidos.
Diante disso, o SUS encontra na novela Abaré um “case” desafiador em todos os sentidos, seja em torno do modelo técnico assistencial (que inspirou estratégias diferenciadas para a realidade amazônica) da universalidade do atendimento, da descentralização dos serviços e da participação social em seu controle.
As oportunidades ainda abertas, fruto de conquistas ao longo desses anos todos através do navio-hospital Abaré, dependem exclusivamente de encaminhamentos e processos decisórios dos atuais entes públicos, assim como da efetividade das instâncias de controle social, podendo ou não se desdobrar em um final feliz.
Pode terminar com um guerra de vaidades entre grupos ou municípios, busca de culpados, mantendo como maiores prejudicados os ribeirinhos sem atendimentos, cuja saúde não pode esperar.
A janela do ministério se fecharia, com a suspensão da Portaria 2.488/2011 no Tapajós, tornando insustentável os serviços assistenciais onde tudo começou, um sinal amarelo também para os outros 50 barcos de saúde em vias de operação pelos interiores de toda Amazônia.
Assim como pode terminar com o bom senso e a soma de esforços de todos os atores deste elenco social -inclusive a TDH- para concretizar a aquisição em definitivo do Abaré, já que o Ministério da Saúde ainda aguarda a deliberação dos municípios.
Com isso, resgata-se o projeto para diversificação dos serviços e programas (barco-escola, saúde do trabalhador etc), agregando novos parceiros potenciais (governo do Estado, órgãos de ensino e pesquisa etc), transformando o Abaré em um polo difusor de boas práticas para as demais unidades de Saúde Fluvial da Amazônia.
Seria um exemplo também de postura política de nossos gestores, que liderariam e sacramentariam o polo Tapajós como referência nacional em saúde ribeirinha.
Mas, no caso desta Novela Abaré, quem a escreve não decide seu final.

naturally like your website however you have to test the
spelling on several of your posts. Many of them are rife with spelling issues
and I find it very troublesome to tell the reality then again I’ll definitely come again again.
