Abaré: 29 anos de lutas e sonhos

Como um dos coordenadores gerais do Projeto Saúde e Alegria (PSA), diante dos últimos acontecimentos sobre o Abaré, acho o momento oportuno também para trazer um pouco do resgate histórico disso tudo.
Sou médico formado pela Universidade Federal do Rio de Janeiro – UFRJ, fiz residência na UFF e vim para a Amazônia para trabalhar no hospital da Universidade em Oriximiná, por ser desde aquela época apaixonado pela região e suas comunidades, e querer atuar em áreas onde poderia ser mais útil na minha profissão.
Cheguei em Santarém em 1984, a convite do então prefeito Ronan Liberal, para atender as comunidades ribeirinhas, na época 800 localidades sem nenhum tipo de assistência de saúde. Muitos comunitários nunca haviam visto um médico, ainda mais vindo até suas casas. Encontramos uma situação drástica com crianças morrendo a rodo por simples diarreias pelo fato de beberem água direto dos rios sem tratamento, ausência de sanitários nos domicílios, anemia e parasitoses, feridas na pele, dentes ruins e infectados, índices de vacinação baixíssimos, etc. Doenças primárias e simples que por falta de ações básicas e preventivas nas comunidades, acabavam por se agravar e levar a um quadro emergencial de saúde na região.
Iniciamos, eu e minha ex-esposa (a arte-educadora Márcia Gama) um enorme trabalho de educação e prevenção em saúde, mobilização dos moradores, construção de fossas sanitárias, tratamento da água, saúde da criança, e treinamento de agentes locais. As respostas foram imediatas e a melhoria na saúde significativa.
Com as mudanças na gestão municipal da época, o trabalho foi interrompido. Para dar continuidade a ação anterior, fundamos então o PSA, que retomou as atividades de forma mais ampla (além da saúde, com programas de educação, geração de renda, etc), a partir de um primeiro financiamento em 1987 do FINSOCIAL/BNDES, interveniência da UFPA e supervisão técnica da FIOCRUZ.
Desde aí, avançamos muito nas campanhas preventivas, segurança alimentar, melhorias no saneamento, etc. mas em relação ao atendimento médico propriamente dito, era algo ainda precário, feito por mim e mais alguns médicos voluntários, com nossas limitadas caixas de medicamentos durante as visitas. Passávamos por grandes “ sufocos” e situações desesperadoras quando aconteciam casos graves e urgentes e estávamos lá no meio do rio sozinhos e sem recursos adequados. Não tínhamos realmente uma boa resolutividade e nosso grande sonho era implementar um barco-hospital com equipamentos e capacidade adequadas. Desenhamos o projeto do barco, muito parecido com o do atual Abaré I. E o BNDES aprovou os recursos para sua implantação.
Até então, éramos uma iniciativa exclusivamente apoiada pelo governo brasileiro, como acreditávamos que deveria ser, pois nossa experiência poderia colaborar para aperfeiçoar os serviços públicos que se desenhavam à época. No entanto, com o “Plano Collor”, o FINSOCIAL foi extinto, e lá se foi o sonho do Barco-hospital, começando um período de muitas dificuldades financeiras para o PSA.
Devagar e sempre – após 3 anos de grande luta garantindo ao menos as atividades mínimas emergenciais que não podiam ser interrompidas nas comunidades por meio de recursos pessoais e ajuda de familiares – fomos conseguindo novos apoios e parcerias para reconstruir tudo novamente, embora de maneira mais sólida e segura.
Em 2005, depois de implantar as ações anteriores e ampliar a área de abrangência do trabalho com ações básicas de saneamento e prevenção em larga escala, chegou a hora de retomar o antigo sonho do barco-hospital. Buscamos parceiros dispostos e redesenhamos toda a proposta de modelo de saúde. O glorioso barco Abaré começou a operar em 2006 em parceria com as Prefeituras, e de lá para cá a historia já é bem conhecida.
Só chegamos onde chegamos hoje como resultado sempre de muita luta, determinação, abnegação e dedicação incansável dos profissionais da nossa equipe , a maioria de Santarém, das comunidades e muitos outros colaboradores, que juntos ajudaram a construir coletivamente este sonho e alcançar os resultados que temos hoje.
Sempre buscamos a parceria com as gestões públicas, independente de qual grupo ou partido estivesse no poder. Apesar dos nossos técnicos pessoalmente terem o direito de livremente definirem suas preferencias políticas, como instituição somos apartidários e também não queremos substituir ou competir com o estado, mas sim desenvolver tecnologias sociais adaptadas, eficientes, de baixo custo e alto impacto, que possam contribuir para aperfeiçoar os serviços públicos, programas de governo e melhorar a qualidade de vida e cidadania da população excluída nestas regiões.
O Abaré foi um exemplo disso, quando desenvolvemos um modelo exemplar para uma das questões mais difíceis na Amazônia que é a saúde das comunidades isoladas. Isto foi adotado como politica pública nacional e repassamos todo o modelo e sua gestão para o município, inclusive com financiamento de longo prazo já assegurado pelo próprio governo federal. Quando adquirimos o Abaré II, repassamos para o município num comodato de 99 anos!
Pronto, nossa missão estava cumprida! O projeto passava a ser novamente uma inciativa exclusivamente nacional, pública, adotada e gerida pelos nossos governos e replicada para outras regiões, exceto pelo detalhe da propriedade do Abaré 1 ainda ser dos holandeses. Mas encaminhamentos avançaram até o final do ano passado para compra do barco pelo Ministério da Saúde, que também assegurou verbas federais complementares para o custeio das atividades.
Vale ressaltar que o custeio do Abaré, por ser um barco grande e de alta qualidade, é realmente elevado, inclusive para os padrões da Portaria. Mas justamente por ter essa estrutura que torna possível funcionar também como barco-escola e incluir outros serviços e programas governamentais, coisa que a estrutura do Abaré II não permite realizar, o que explica o interesse do Ministério da Saúde na sua aquisição.
Bem, quero ressaltar o grande respeito que tenho pela atual equipe da SEMSA e o testemunho de seu sincero empenho e preocupação com a saúde dos ribeirinhos e com a manutenção do Abaré. A própria Secretária assim que assumiu em janeiro, viajou como médica atendendo no Abaré, para aliviar a demanda atrasada.
Mas quero observar que com a atual opção adotada pela Prefeitura de manter um contrato por um ano de aluguel do barco por 10 dias/mês (ou mesmo que renegocie para cessão do barco sem precisar pagar), voltamos a uma situação anterior que já havia sido superada e se mostrado bastante insegura e arriscada, que é um formato que depende de uma entidade estrangeira, de renovações e renegociações anuais para permanência do barco, que já vimos que pode mudar de uma hora para outra.
Acho também que nossa missão enquanto PSA está realizada e com bastante mérito. Os Abarés e o modelo já estão plenamente entregues a gestão municipal e a sociedade de Santarém. A saúde é responsabilidade pública e um sistema participativo. Cabe agora as comunidades, a população, e ao Conselho Municipal de Saúde – que tem se mobilizado em busca de soluções – continuar a lutar pelo que é seu, e serem tomadas decisões que são exclusivamente de governo, tanto os Municipais, como o Ministério da Saúde.
