Abaré: interiorização da medicina. Por que perdê-la?

O debate sobre o Abaré I, que se tornou referência nacional de saúde nas regiões de rios da Amazônia, deve ser entendido não como um problema, mas como uma forma de mobilizar oportunidades para a interiorização da medicina na nossa região, como uma política pública integradora de atendimento e de ensino na área de saúde.
A discussão já vai longe e me manifesto trazendo à tona alguns elementos que considero importantes, como médico que vivenciou essa experiência por 4 anos, e atualmente como coordenador adjunto do curso de medicina UEPA – Universidade Estadual do Pará.
A construção do modelo de atendimento em saúde aos ribeirinhos por meio do Barco Abaré I trouxe não apenas a melhoria das condições de saúde da população que vive distante dos centros urbanos, que de fato sempre foi seu principal objetivo, mas também resultou em referências que devem ser levadas em consideração no atual contexto.
Especialmente num momento em que o Brasil discute a necessidade de interiorização dos médicos, dada a carência nas pequenas e médias cidade, principalmente no nordeste e norte do país, e mais ainda na realidade das zonas rurais.
A primeira delas é o fato de que, além do trabalho de atenção básica de saúde realizada no Abaré I, foi também possível, pelo caráter inovador do projeto, mobilizar diversas parcerias, para a realização de jornadas de cirurgias, que tiveram impacto positivo na demanda reprimida de patologias simples que hoje impactam o sistema de saúde, na área de oftalmologia (cataratas e pterígios), odontologia (caries e próteses) e hérnias.
Somente entre 2009 e 2011, foram realizadas uma média de 4 jornadas, com a realização de 600 cirurgias. Isto demonstra a capacidade que um projeto como esse tem ajudando na mobilização e atração de profissionais médicos para a região, mesmo que não de forma permanente, mas contribuindo para diminuir a grande demanda existente.
Outro aspecto relevante é que a infraestrutura do barco, com laboratórios, farmácia, consultórios médicos, de enfermagem e odontologia, permitia não apenas a atenção básica adequada ao controle da saúde dos ribeirinhos incluídos nos programas de Saúde da Família, com impactos positivos em vários indicadores de saúde como vacinação, desnutrição, aleitamento materno, aderência nos programas de hiperdia, PCCU, e pré natal.
Esta estrutura permitia também dar suporte aos programas de prevenção, capacitação de agentes comunitários de saúde, e ao receptivo de residentes médicos à bordo. As parcerias com as universidades possibilitaram o desenvolvimento de linhas de pesquisas e trabalhos científicos que envolviam sempre a visão da academia voltadas para a solução de problemas de saúde comunitária, o que chamamos de Trabalhos científicos de base comunitária- com realização de varios TCC, TCA e teses.
Além disso, com essas parcerias, conseguíamos ter a presença de mais profissionais além dos raros médicos oriundos das secretarias de saúde que se dispunham a este tipo de trabalho. Além do médico chefe das equipes, contávamos sempre com médicos residentes e alunos do quinto ao sexto ano de formação e enfermeiros oriundos não só das universidades locais, mas também de outras universidades, como a USP, UFSC, UNB, UFPB, UFPA; UNICAMP etc.
Portanto, tratava-se de um verdadeiro estágio de interiorização que expandia a capacidade de atenção e a qualidade nos atendimentos, como observamos hoje no Hospital Municipal de Santarém, a grande contribuição de internos e médicos residentes supervisionados, frente à grande demanda existente melhorando a qualidade do atendimento.
Com isso conseguimos ter uma resolutividade de 93%, ou seja, somente 7% dos pacientes precisavam se deslocar até a cidade, pois a maioria dos problemas eram resolvidos ali mesmo à bordo. Isso portanto, significava um atendimento de qualidade, resoluto, que além de tudo, ainda diminuía a pressão ou aumento dos pacientes nos postos e hospitais já saturados da cidade.
A UEPA formou no ano passado sua primeira turma de médicos. Sempre acreditamos ser fundamental permitir aos estudantes de medicina que pudessem ter uma formação também voltada às demandas das populações rurais. Ano passado foi criado na UEPA o Programa de Residência Médica em Saúde da Família . O programa de saúde da família fluvial possibilita que os médicos residentes tenham a chance de aprender como praticar a medicina nas áreas ribeirinhas.
É neste ponto, como coordenador adjunto do curso de medicina da UEPA, que vejo uma grande perda, tanto para as comunidades, quanto para nossos alunos, e também para as outras universidades parceiras e associações médicas, que poderiam continuar atraindo oportunidades de oferta de serviços de saúde para nossa região.
Olhando o debate hoje no Brasil, do desafio de interiorização da medicina, temos um modelo que vinha se demonstrando efetivo para as áreas ribeirinhas, pois unia condições de trabalho, e uma forma inovadora e atrativa, com a formação de equipe de saúde voltadas às comunidades às margens dos rios.
O modelo gerado e desenvolvido aqui no Tapajós já está sendo replicada através da portaria ministerial 2191 em outros Municípios da Amazônia Legal e Mato Grosso do Sul. No entanto, aqui estamos vendo toda essa construção ser conduzida de uma forma reducionista, sem levar em conta todo seu potencial.
A falta de uma solução definitiva por parte do poder público municipal, para a permanência do Abaré I no Tapajós, ficando refém do aluguel do Barco, inviabiliza a continuidade de todo esse movimento. Até mesmo a regularidade da presença do barco nas comunidades, responsável pela melhoria dos indicadores de saúde, começa a ser prejudicado. Apenas em maio o Abaré I fez sua primeira viagem e algumas comunidades reclamam da ausência de médicos à bordo.
Se não houver uma solução definitiva para a permanência do Abaré I no Tapajós, aproveitando as oportunidades manifestadas pelo Ministério da Saúde para sua aquisição, poderá ser uma perda para todos. Para as comunidades, as universidades, e a política municipal de saúde. Porque perder essas oportunidades?
