Santarém, Belterra e Aveiro recebem primeira equipe de Saúde da Família Fluvial do Brasil

As comunidades ribeirinhas dos municípios de Santarém, Belterra e Aveiro receberam nesta última terça-feira, 07/12 em Santarém, a primeira Unidade de Saúde da Família Fluvial do Brasil, um projeto inédito no contexto das políticas de saúde no país, resultado da portaria ministerial 2.191 de 3 de agosto de 2010.
À bordo do navio Abaré, a cerimônia contou com a presença da Prefeita de Santarém, Maria do Carmo, do Secretário Municipal de Saúde de Santarém, José Antonio Rocha; o prefeito de Belterra, Geraldo Pastana; a presidente do Conselho Municipal de Saúde de Santarém, Maria das Dores Colares; o presidente da Camara de Vereadores de Santarém, José Maria Tapajós; o enfermeiro Marco Aurélio, representante da prefeitura de Aveiro; Eugênio Scanavino, fundador e coordenador geral do Projeto Saúde & Alegria; Critianne Haraki, representante nacional do Terre dês Hommens Holanda, e a médica Claunara Schillig Mendonça, diretora do Departamento da Atenção Básica (DAB) do Ministério da Saúde.
A iniciativa toma como referência o projeto demonstrativo do Barco-Hospital Abaré no Oeste do Pará desenvolvido pela ONG Saúde e Alegria em parceria com as Prefeituras de Santarém, Belterra e Aveiro e apoiado pelo Terre dês Hommes Holanda.
A partir dessa experiência e como resposta a demanda antiga das comunidades amazônicas, o Ministério da Saúde lançou a portaria que instituiu critérios diferenciados para implantação, financiamento e manutenção da Estratégia de Saúde da Família para as populações que residem às margens dos rios através de Unidades de Saúde da Família Fluviais, que podem utilizar barcos e equipes de trabalho que necessitam se deslocar para atender comunidades em áreas remotas.
Um rio de desafios para promover saúde na Amazônia
Na Amazônia, com suas características geográficas, culturais e sociais peculiares, são muitos os desafios para promover a saúde. Longas distâncias, dificuldades de transporte e comunicação, além de precárias condições de saneamento geram o grande desafio da inclusão das comunidades ribeirinhas aos serviços básicos de saúde.
Alguns indicadores, expõe essa realidade, como no caso da interiorização da medicina. Partindo da premissa que a Organização Mundial de Saúde (OMS) preconiza como parâmetro ideal de atenção à saúde a relação de 1 médico para cada 1.000 habitantes, na região amazônica, mais que em qualquer região do país, ainda observamos grandes distorções. A Amazônia legal apresenta relação média de 1 médico para cada 1.249 habitantes, abaixo da média nacional de 1/622. Além disso, deste total, a maioria está concentrada nas áreas urbanas e peri-urbanas. A demanda se torna muito maior nas zonas rurais e comunidades ribeirinhas. Nas zonas rurais do Pará, a proporção é a maior do país, sendo de 1 médico para cada 4.466 habitantes. (fonte: portalmedico.org.br).

Daí a importância desta iniciativa, pois muitas vezes essa realidade é fruto da falta de financiamento adequado. “Nós temos um modelo nacional, que são as equipes de saúde da família, mas temos características muito diversas no Brasil. Então, é um avanço imenso essa portaria, porque ela garante a especificidade das populações ribeirinhas, de um custo diferenciado, de uma forma de atendimento e um processo de trabalho também diferenciados de quem está numa unidade fixa nas cidades. Essa é a primeira de uma série de ações que visam ampliar ainda mais a população coberta pela Estratégia Saúde da Família (ESF). O intuito é tornar a atenção primária acessível em todo o território nacional, superando qualquer obstáculo, inclusive os geográficos”, afirmou a Dra. Claunara Mendonça, diretora do DAB do Ministério da Saúde.
O Dr. Fabio Tozzi, um dos coordenadores do projeto, completa que “ a importância é que trata-se de um aprimoramento do SUS, sem dúvida uma das principais conquistas da saúde no Brasil, mas que ainda tem muitos desafios, principalmente pelas desigualdades sociais e geográficas do país. Mas o desenvolvimento de uma experiência como essa comprova que é possível aprimorar o sistema para que ele chegue efetivamente na ponta, para quem mais precisa”.
Experiência resulta de parceria entre poder público e sociedade civil
Parceria. Esse foi o tom do discurso dos representares envolvidos na iniciativa, que envolveu o poder público e a sociedade civil. Para Cristianne Haraki, representante no Brasil da Terre des Hommes Holanda, um dos principais parceiros da iniciativa, “desde o início da parceria com o PSA buscamos proporcionar as populações do Rio Tapajós uma assistência de qualidade e alinhada com as políticas preconizadas pelo Ministério da Saúde. Faltava este reconhecimento. Como organização comprometida em melhorar da qualidade de vida principalmente de crianças e adolescentes, manifestamos nossa satisfação pela iniciativa de inserção da experiência do Barco Abaré a política de saúde do Brasil”, afirmou.
A prefeita de Santarém, Maria do Carmo, elogiou o empenho do Saúde e Alegria e demais parceiros pela aprovação da portaria. “As organizações da sociedade civil, as ONGs são como faróis da sociedade, que entendem as demandas onde o poder público por alguma razão não consegue chegar, abrindo caminhos e novas possibilidades. É o caso desse projeto ao qual nos juntamos desde o início da nossa gestão. A concepção de saúde pública é atender as pessoas onde elas estão, nas comunidades. E o PSA é um especialista em atendimento às comunidades. Aqui portanto, é a junção vitoriosa dessas concepções”.
O Prefeito de Belterra, Geraldo Pastana, também comemorou a iniciativa. “Precisava que o modelo do barco Abaré fosse incorporado numa política pública, para que o município tivesse condições de assumir sua responsabilidade, e que pudesse também ser levado como experiência para outras regiões da Amazônia. Essa é a satisfação maior com essa portaria, que vai proporcionar a saúde ir nas comunidades mais distantes”, afirmou. Uma visão também compartilhada pelo representante da Prefeitura de Aveiro, Marcos Aurelio. “Poderá servir de exemplo para outros municípios promoverem a atenção básica nas comunidades mais distantes”.
“Esta é uma uma vitória importante, pois representa um dos principais objetivos do Saúde e Alegria que é contribuir para o aperfeiçoamento das políticas públicas na Amazônia, sobretudo junto às populações mais isoladas. Além de ser um ótimo exemplo de parceria do terceiro setor com o poder público, o que se semeou a partir da experiência no Tapajós poderá gerar frutos para toda Amazônia, estendendo os benefícios a um numero muito maior de pessoas”, afirma o coordenador geral do Projeto Saúde e Alegria, Caetano Scannavino.
20 anos levando saúde e alegria às comunidades da floresta

A história de mais de 20 anos atuando nas comunidades ribeirinhas da amazônia foi lembrada durante o evento da última terça-feira. O médico Eugenio Scannavino Netto lembrou do tempo em que começava a organizar o seu Projeto Saúde e Alegria. Com um pequeno barco que cabia pouco mais de 05 pessoas, realizava principalmente um trabalho de educação e prevenção. Mas com as poucas condições, faltava recursos para realizar os atendimentos para os muitos casos de doença encontrados na época.
Um barco estruturado para atendimentos médicos era apenas um sonho que, com muito esforço e dedicação de uma equipe incansável, poderia se tornar realidade em 2006. Foi quando o atendimento básico de cerca de 70 comunidades do rio Tapajós começou a ser feito através do Barco Abaré, beneficiando mais de 15 mil pessoas. Hoje elas têm acesso regular à bordo do posto flutuante, de 40 em 40 dias, aos atendimentos médicos e odontológicos, vacinações, procedimentos laboratoriais, pequenas cirurgias, além exames preventivos como o pré-natal e o PCCU, entre outros serviços.
Ações de educação e prevenção complementam este modelo com a realização de Campanhas Educativas acompanhando as visitas do barco. Arte-educadores apresentam o Circo Mocorongo com teatros e brincadeiras, repassando conhecimentos de como evitar as doenças e promover saúde com alegria.
O mais importante foram os resultados obtidos com a chegada do Abaré:
> 96,5% das crianças estão vacinadas (indicador superior à média estadual, que é de apenas 83,3%);
> 2% das crianças menores de 2 anos estão desnutridas (nas comunidades não atendidas, a taxa é de 5%);
> 90% das crianças de até 6 meses são alimentadas exclusivamente com o leite materno;
> 98% das mulheres grávidas fazem o pré-natal (apenas 73% no restante do Estado)
“Nós sempre lutados por uma saúde de qualidade para as populações ribeirinhas. E acho que neste momento, com o lançamento dessa portaria, estamos cumprindo uma das principais missões da nossa organização, que é ter contribuído com tecnologias sociais para aperfeiçoar as políticas públicas de saúde, o que vai beneficiar um número muito maior de comunidades”, comemorou Eugênio Scannavino.
Gestão e controle social
Autoridades assinam convênio
No evento foi assinado o contrato que torna o Barco Abaré a primeira Unidade de Saúde da Família Fluvial, que a partir 2011 poderá ser financiado também pela política pública de saúde, capitaneado pela SEMSA de Santarém, que receberá 40 mil reais por mês. O Projeto Saúde e Alegria continuará seu trabalho demonstrativo e responsável pelas ações de educação, prevenção e controle social do modelo.
O Secretário Municipal de Saúde de Santarém, José Antônio Rocha, disse que 2011 será ainda um ano de experimentação, mas que a Unidade já começa a funcionar. “Santarém será o município pólo e vamos trabalhar em consórcio com os municípios de Belterra e Aveiro”.
A proposta vai ser acompanhada pelo Conselho Municipal de Saúde Santarém. Sua presidente, Maria Colares, tem ótimas expectativas. “Nós conhecemos as dificuldades dos nossos ribeirinhos. Se aqui na cidade as vezes existem dificuldades, elas são muito maiores no interior. Estando o médico mais perto, isso vai facilitar muito a vida dessa população. Nós avaliamos muito e apoiamos esse projeto, porque ele tratá ainda melhores resultados”.
O representante da Organização das comunidades da Reserva Extrativista Tapajós/ Arapiuns, morador da comunidade de Suruacá e usuário do modelo, Leônidas Bentes, também avalia positivamente a iniciativa. É muito importante porque vai continuar e aperfeiçoar o trabalho que já é feito pelo Abaré nas comunidades do Tapajós e que mudou muito a realidade da saúde das comunidades, diminuindo muitas doenças que ocorriam antes. Para nós a saúde era uma das maiores preocupações. Agora nós temos um atendimento de qualidade. E estamos felizes porque outras comunidades poderão se beneficiar disso também”.
